Almost every timeline you’ll read was written for a well-fed 25-year-old. This one is for an older beginner eating much less than usual — what actually changes in week two, week six and month three, and which of those changes you can expect to notice.

Strength and size run on two different clocks

These are separate things and they arrive at different times, which is the single most useful thing to understand about the first few months. Strength is partly skill: how well you recruit what you already have, how stable the movement is, how confident you are going down into a squat. That improves quickly, because it is learning rather than building, and it is available at any age.

Muscle itself is slower. It is tissue, it has to be assembled from material you eat, and on a GLP-1 you are eating less of everything. So the honest expectation is a strength curve that moves in weeks and a size curve that moves in months — and for many people on a medication, a size curve whose success looks like a flat line rather than a rising one.

A realistic week-by-week

Weeks one and two are for learning the movements and finding out what your joints think of them. You may be sore, and soreness tracks novelty rather than progress, so there is nothing here worth judging yet. The only goal is to do the sessions and finish them able to walk normally the next day.

Weeks two to four are usually where the first real signal shows up: the same workout takes less out of you, and you get an extra rep or two at the same weight. That is progress, and it is the kind that arrives before the mirror does.

Weeks four to eight are where load starts to move. A long-standing ACSM guideline suggests increasing load by roughly 2–10% once you can perform the current workload for one to two repetitions beyond the number you were aiming for — a small, specific rule that keeps you moving without turning every session into a test.

Months two and three are the earliest point at which a scan or a photograph has anything useful to say, and even then the change has to be bigger than the measurement error before it counts. If your lean mass is roughly where it started while your weight has come down, that is the outcome the training was for.

What the GLP-1 changes about the timeline

Mostly the size half, and mostly through intake. Building tissue takes protein and fuel, and a small appetite delivers less of both — so progress tends to be flatter than the same programme would produce on a normal appetite. That is not a sign the training is failing; it is the arithmetic of a deficit, and the training is what keeps the deficit from taking muscle with it.

Why strength and lean mass can disagree

They are not the same measurement and they do not have to move together. In a randomised trial in postmenopausal women, groups losing weight at very different rates lost different amounts of lean mass yet showed no difference in handgrip strength — a reminder that a number on a scan is not the same thing as what you can do.

That cuts both ways, and it is worth knowing before you get a result that confuses you. Lean mass can drift down while your strength holds or improves, because you are getting better at using what you have. It is one more reason to keep a training log rather than relying on a scan you take twice a year.

What to measure so you can actually see it

Pick two or three things and write them down, because memory is generous and specific numbers are not. Reps at a fixed weight is the simplest. How many sit-to-stands you manage in thirty seconds is another, and it needs no equipment. So is the plain question of whether a flight of stairs still requires a plan.

When it isn’t a plateau

Progress stalling for a few weeks is ordinary, particularly while you are eating much less. Going backwards is different. Strength that is clearly declining, unusual weakness, dizziness or faintness during sessions, or an ache that outlasts a few days are all worth raising with your healthcare provider rather than training through — and that goes double if anything about your medication or dose has recently changed.
