Muscle or Water Weight? Reading the Scale on a GLP-1
The short answer
Early on, mostly water. Weight that disappears in the first week or two comes largely from stored glycogen and the water held with it, plus less food sitting in your digestive system at any moment — none of that is muscle, and some of it returns the day you eat a large meal. Muscle loss on a GLP-1 is real, but it moves over months, and the bathroom scale is the one tool that cannot show it to you. Time is what separates them: water swings day to day, muscle changes across months.

What the research does say clearly is that whatever the pace, protein and resistance training shift what the loss is made of.

Why the first two weeks move so fast
Your body stores carbohydrate as glycogen, and glycogen is held in the body together with water. Eat considerably less, which is what happens on a GLP-1, and that store draws down and takes the water with it. Alongside that there is simply less food in transit through your digestive system at any given moment. Both of those resolve within days, and neither one is fat or muscle.
This is why the opening drop outpaces everything that follows, and why it predicts almost nothing about the months after it. When the rate slows down, nothing has gone wrong — the water-shaped part of the loss was only ever going to happen once.
‘Lean mass’ is not a synonym for muscle
When a scan reports lean or fat-free mass, it is reporting everything in you that is not fat: muscle, but also organs, bone mineral and a great deal of water. Fat-free mass is roughly 74% water, 19% protein and 6.5% mineral, and researchers estimate only around 60% of fat-free mass lost during weight loss comes from skeletal muscle, with much of the remainder being water within muscle.
So a scan showing lean mass down two kilos is not two kilos of muscle, and how hydrated you were on the morning of the scan is part of the reading. If you want that number to mean something, take it under the same conditions each time and compare across months rather than weeks.
Short-term swings are mostly not fat either
In a study of free-living adults, around 84% of the small average weight change measured over two weeks was fat-free mass rather than fat — short-term swings are dominated by water and glycogen. That is the measured version of what everyone notices anyway: a salty dinner, a short night, a day of drinking less, and the scale reports something that has nothing to do with your body composition.
The practical consequence is that a single weigh-in carries close to no information about tissue. A weekly average across a month carries some. Anything you conclude from Tuesday against Monday is noise with a decimal point on it.
What would actually tell you about muscle
Your training log, mostly. Muscle you are holding onto shows up as reps and load staying level or climbing while your weight comes down. Muscle you are losing shows up as the same sessions getting harder over a span of weeks rather than days. It is a slower signal than the scale, and it is the one that is genuinely about muscle.
A tape measure at fixed points and photographs taken in the same light are also more informative than daily weighing. And whichever way those go, the response does not change: resistance training a couple of times a week, and enough protein. Those are the two things that move how the weight splits.
When it is not a measurement question at all
Losing fluid is not always harmless. Dizziness on standing, cramping, not being able to keep fluids down, or weight coming off considerably faster than you and your prescriber expected are matters for your healthcare provider rather than things to interpret from a graph.
The same is true of real weakness — not the ordinary flatness of eating much less, but strength that is clearly heading backwards. They can look at the whole picture, including the parts a bathroom scale has no access to.

Am I losing muscle or water weight on a GLP-1?+
In the first days and weeks, mostly water. Eating much less draws down your stored glycogen, and glycogen is held in the body along with water, so that share of the loss leaves quickly and can return just as quickly. Muscle loss on a GLP-1 is real but slow, measured across months — which makes the earliest, fastest part of the drop the least likely part to be muscle.
Why did I lose 3 kg in week one and then almost nothing?+
Because week one included something that does not repeat: the water stored with your glycogen, and less food sitting in your digestive system. The slower rate afterwards is the actual trend. It is not a sign that the medication has stopped working or that you have done something wrong.
Is the scale useless, then?+
As a daily reading, close to it — in one study of free-living adults, around 84% of the small average weight change over two weeks was fat-free mass rather than fat. As a weekly average watched over a month or two it is perfectly serviceable. What it cannot do, at any frequency, is tell you which tissue you are losing.
My scan says lean mass dropped. Is that all muscle?+

Your hydration on the morning of the scan is part of the reading, which is why one scan is weaker evidence than a training log covering the same period.
Should I drink less water so the scale moves?+
No, and it is worth saying plainly because the idea is common. You would be moving the number without changing anything about your body, and on a GLP-1 — where eating and drinking less is already the situation — running short on fluids is the more realistic problem.
