Is BMI Accurate on a GLP-1? What It Can and Can’t Tell You

The short answer
BMI is a screening tool built for populations, not a readout of your body. It is your height and your weight and nothing else, so it cannot distinguish muscle from fat — which is exactly the distinction that matters while you’re losing weight on a GLP-1. In a study of 13,601 US adults, a BMI of 30 or above was highly specific but poorly sensitive against measured body fat: it missed roughly half or more of the people whose body fat fell in the range it was meant to flag. Treat it as a coarse starting point your clinic may use, and track waist and strength for the question BMI structurally can’t answer.
BMI is the number you get handed most often and the one that tells you least about what’s happening to your body composition. Here’s what it’s actually for, where it breaks, and what to put beside it.
What BMI actually is
BMI is weight divided by height squared. That’s the whole calculation — no measurement of fat, no measurement of muscle, no measurement of where anything sits on you. It became standard because it’s free, instant, and identical everywhere, which makes it genuinely useful for comparing large groups of people.
That population-level job is the one it does well. The mistake is carrying it across to an individual and reading it as a statement about your body, because at that level the thing it can’t see — what your weight is actually made of — is the entire question.
Why it can’t answer the GLP-1 question
On a GLP-1 the question isn’t whether your weight is falling; it’s what’s coming off with it. BMI is structurally incapable of separating those. Two people at the same BMI can have very different amounts of muscle, and the same person can lose lean tissue while their BMI drops in a way that looks like unambiguous progress.
The research bears out how loose the relationship is. In 13,601 adults from a US national survey, a BMI of 30 or above was highly specific against body-fat measurement (95% in men, 99% in women) but poorly sensitive — 36% in men and 49% in women. And in the middle band, a BMI between 25 and 29.9, BMI failed to discriminate between body fat and lean mass in either sex. That middle band is where a lot of people on a GLP-1 spend months.
What to put beside it
Waist circumference is the obvious companion, and not as a consolation prize. An international expert consensus concluded that waist circumference carries information that is both independent of and additive to BMI, and recommended measuring it routinely rather than relying on weight alone. The second companion is your strength trend, because it’s the only one of these that reflects muscle directly rather than inferring it. Where BMI is still worth having
It would be overcorrecting to call it useless. Clinical thresholds, eligibility criteria and a great deal of research are written against BMI, so it stays part of the vocabulary whether or not it deserves to be. Your clinic may use it to monitor you, and that’s a reasonable thing for them to do with a cheap standardised number.
What it shouldn’t be is your progress measure. And whether your own BMI means anything for your health, or whether it should change what you’re doing, isn’t a question a calculator or a web page can settle — that belongs with your healthcare provider, who can look at the rest of the picture.

It works less well, and in a predictable direction. Muscle is denser than fat, so building or holding muscle adds weight without adding fat — and BMI reads that as the same thing as any other weight. Someone strength training through a GLP-1 can see their BMI move slowly while their body composition improves. That’s a limitation of the formula, not a sign anything is going wrong.
Should I stop tracking BMI on a GLP-1?+
You don’t have to drop it, but don’t let it be your progress measure. Add a waist measurement, which expert consensus found adds information BMI doesn’t carry, and a strength number, which is the only one of the three that reflects muscle directly.
